Anyone?
I know he is in training as a squire to become a knight. In the Smurfs and the Magic Flute, Johann has achieved his Knighthood. So the question is, when did that happen? Who sponsored him? Was he from another Kingdom?
One more question, how far away was King Gerard's Kingdom from "The Good King's" Kingdom?

Unfortunately, not much is known about Johan's background and it does not help that most of the "Johan and Peewit" comic stories remain untranslated (it was never popular outside the French-speaking world). The same goes with the origins of the Smurfs, such as why they are naturally males, or why they come to the world through a stock as a baby during a magical blue moon.

Should it really matter? Sometimes there are things that shouldn't be addressed.

In an idea that I had for an Empath fanfiction story where Empath travels to the present day and meets up with King Gerard's desecendant, I was wondering what surname his descendant would go under. I was thinking something like Jerry Royeaux for the descendant.

Not much is known about Johan's background so I can only make conclusions based on the cartoon, the book series and historical information regarding knighthood.
Johan is most likely the son of a knight and from an aristocratic family that is very close and loyal to the king, in order for the king to make Johan his very own squire. In the books, Johan is shown on several occasions helping the king get into his armor.
When the time came for Johan to be knighted, I believe the king would sponsor him since Johan is his squire.
Johan has always had the title of squire in the cartoon and books but in the book series his fighting skills seem to progress in each story as well as his personality seems to mature.
I don't know how far apart King Gerard's and the good king's kingdoms are but if I had to guess I would say they are at each end of the forest. In the cartoon, King Gerard and Princess Sabina are cousins so I guess the good king would be King Gerard's uncle.
Hope this helps somewhat.
